Professional ServiceM8 forms. No Word required.

Why

One design. The questions and the PDF.

A ServiceM8 form is two builds today: the questions in ServiceM8, and a Word template full of field codes. Change one and the other breaks. Form Designer makes both from one design, so the fields always match. You download one form file and import it.

Three starter forms as they print: a Job Completion Report, a Job Safety Analysis and a Vehicle Pre-start Check
Sound familiar?

Stop fighting Word. Start designing forms.

Until now the choice was to pay someone for each form, lose your own hours in Word, or put up with a form that looks amateur.

“I paid for a form and now I cannot change one line myself.”

Open a form you already use, change it and download it again the same day.

“The PDF looks nothing like our brand.”

Your logo, your colours, your fonts. Apply a look copies them onto every form.

“One wrong field code and the whole form breaks.”

You never type a field code. The questions and the PDF come from one design.

“The technician has to scroll past questions that do not apply.”

A rule hides what does not apply on the job. The same parts stay off the PDF.

“I do not know where to start with a blank page.”

Start from one of eight finished forms. Each has its questions, its rules and its page design.

“Our forms all look different from each other.”

Pick one look and put it on every form, also on a form you imported.

How it works

Four steps. One form file.

Build the form in your browser, inside ServiceM8. When it looks right, download it and import it: Settings, Forms, Import.

01 · START

Start from a form that works.

Eight starter templates, each with its own look. Pick one, add your logo and change what you want. Or open a form you already use in ServiceM8.

02 · LAYOUT

Drag the page together.

Headings, tables, sections, your logo, questions and ServiceM8 job details. Drop them on an A4 page at true size. No field codes to type.

03 · QUESTIONS

See which answer decides what.

The Logic map draws a line from each question to the ones it shows or hides. The Logic check finds a rule that can never match, before a tech does.

04 · PREVIEW

Fill it in before the crew does.

Pick sample answers, or run the form one question at a time. The page fills in like the finished PDF. The Workflow shows what was asked and what was skipped.

And the rest

Everything else on the form.

  1. Rules in plain words

“Show this when Hazards is Other.” Put a rule on a question, a section or a table row. Hidden parts are skipped in the app and left off the PDF.

  2. Ticks, crosses and warnings

Yes prints a tick, No prints a cross, N/A prints a dash. Choose the icon and the colour for each answer.

  3. A risk matrix that fills itself in

Two choice questions go in. The result word comes out in its own colour: Low, Medium, High.

  4. Sentences that print only when they apply

Write one line of text for each answer. The PDF prints the line that matches and leaves the others out.

  5. Photo pages built for you

Up to 40 photos, 4 or 9 to a page, with optional captions. Only the photos that were taken print.

  6. Job details that fill themselves

About 80 ServiceM8 details: client, job, contacts, staff, your business and your own custom fields.

  7. Open the form you already have

Open a form file you already use. The questions, the rules and the Word layout come in. Tidy a converted form and Apply a look do the cleanup.

  8. A page that looks designed

Colour bands across the top and bottom, rounded cards, a different first page, money formats and coloured result words such as PASS or DO NOT DRIVE.

  9. One look across every form

Apply a look copies the colours, fonts, header and footer of one form onto another. The eight starter looks are ready to pick.

  10. Your forms stay yours

Designs are saved in your own ServiceM8 account, with earlier versions. Every form is locked: it cannot be changed or exported in ServiceM8, and it prints only in your account.

Starter templates

Eight starter forms. Eight looks. Your logo.

Each one is complete: the questions, the rules and a finished page. Add your logo, change the colours and the words. Any look can go on any form. Each starter can also be bought on its own: see Plans.

Plans

Build for free. Pay when you download.

Install it, build a whole form and preview it with no plan. When you download your first form, you subscribe, or you buy that one form if it is one of the eight starter forms. One subscription covers everyone in your ServiceM8 account.

Install it in ServiceM8.

Form Designer installs straight into your ServiceM8 account. It's free to install, and to build and preview a whole form. No credit card is needed until you subscribe or buy a form.

Worth knowing

A few things to be aware of.

Honest disclosure of the technical caveats. Mostly common-sense, worth flagging up front.

You design on a computer

Form Designer opens from the Add-ons menu in ServiceM8 on the web. It needs a full screen, so it does not run on a phone. Field staff don't need it: they fill in the finished form in the ServiceM8 app as usual.

You import the form yourself

ServiceM8 does not let an add-on install a form. You download one form file (.sm8f) and import it in ServiceM8: Settings, Forms, Import. To change a form later, open it in Form Designer, edit it, download it and import it again.

ServiceM8 makes the final PDF

The Preview is built from the real template, but ServiceM8 prints the finished PDF itself. Text can run a little wider there, so a line can wrap or a page can break a row sooner. Complete the form on a real job before you use it for real.

Your forms are protected

A form you download cannot be changed or exported inside ServiceM8, and it prints only in your own ServiceM8 account. You change it in Form Designer. If your business name changes, download the form again.

Your designs stay in your ServiceM8 account

Designs are saved to My forms, in your own ServiceM8 account. None are kept on WorkM8's servers. Earlier versions are kept, and a deleted form can be restored.

The starters are starting points

The eight starter templates are general examples. They are not made for one trade, and they are not legal or compliance documents. Change the questions, the wording and the conditions to your own rules.

Some forms cannot be opened

A form that was locked outside Form Designer cannot be opened. One example is a form from the ServiceM8 Form Store.

Who can subscribe or buy

Only the business owner can subscribe, buy a form and manage billing. Plans are billed once a year; the Stripe checkout shows the price and tax in your currency. A starter form bought on its own is AUD $149, one payment.

One starter form, bought on its own

Only the eight starter forms can be bought on their own. A form from a blank page, or a form you import, needs a plan. You can change the logo, the colours, the layout and the questions. The form's title, header and footer keep their words, one or two parts of the page stay as they are (for example the Result card of the Inspection Checklist), and each page says “Made with Form Designer”. The preview shows all of this before you pay. My forms keeps one copy of the form, with its earlier versions. Before paying you accept the WorkM8 Software & Services Agreement: the form is for use on your own ServiceM8 system, and it can only be edited in Form Designer.

If your plan ends

Forms you have already imported keep working. Your designs still open. You cannot download a changed form until you subscribe again.

Limits ServiceM8 sets

A choice question holds up to 10 answers. A question holds up to 3 rules, counting the ones from its section and table row. A rule can only use questions above it.
