Yes. You see every page, and you can try the form with your own answers. It is free to connect, build, change, preview and try. The pages have a watermark until the form is paid for. The payment is for the download.
It will ask the same questions, with the same words, in the same order. The layout is clean and new, in the look you pick. It is not a copy of your page. If you need it to look exactly like your PDF, you'll need a custom form build. Learn more here.

Yes. An AI builds the form, and it can make mistakes. Before you pay, read every page and run the form with different answers. After you import it, try it on a test job before your technicians use it.
Your logo and your colour. Ticks, crosses and dashes for answers. Questions shown only for some answers. Photos and a signature. Job, client and business details that ServiceM8 fills in. A risk matrix result. Sentences and rows that print only when they apply. The last three kinds can add to the price.
No. You pay once for each form.
Your logo and your colour, yes. Your own pictures, no: only the logo. Form Builder uses its own icons, such as ticks and warning signs.
Yes, and only in your account. The form prints only there. It cannot be exported from ServiceM8 or changed in ServiceM8’s own form editor, and you cannot give it to another business.
Form Builder is a website. There is nothing to install. You download one form file and import it in ServiceM8: Settings, Forms, Import. Form Builder does not put the form into ServiceM8 for you.
They stay with the old form. The new form is a new form in ServiceM8.
Yes. Save each form you download in a place of your own, and each changed version too. “My forms” lets you open a form again and download it again, but Form Builder is not a store for your forms.
A PDF, a photo (PNG or JPEG) or a form file from ServiceM8: up to 3 files in one message. Upload an empty form, not a filled-in one. A form from ServiceM8 can have up to 120 questions. A locked form is refused, unless Form Builder or Form Designer made it for your own account.
The name of your business and the id of your account. Not your jobs, your clients or your staff. Your forms, your chat and your uploads are kept on our server in Sydney, Australia. Your chat messages and uploaded files are sent to the AI service that builds the form (Claude, from Anthropic, a company in the United States), so they can be processed outside Australia. The rules say what we keep and where it is processed.
That check is yours. Form Builder builds the form you ask for. It gives no legal, safety or compliance advice.
Yes. You can have 5 forms that are not paid for at one time. A form that is not paid for, and has no change for 30 days, is removed. A form can have up to 25 chat messages before payment. There is also a fair-use limit for each account.
You see the whole form before you pay, so a payment is not refundable, except where the law requires it. If the form you download has a fault, we correct it. The full rules for payment and refunds.
